NEET Counselling Documents: Prepare the Right Set

For NEET counselling, keep your documents in three folders: (1) your main records, (2) proof for any special seat you are claiming, and (3) the forms your allotted college asks for. This way, a college form does not get lost among your school certificates on reporting day.

Use the lists below to prepare early. The current counselling authority and your allotted college decide the final list for you.

On this page

Folder 1: Your main records

Almost every student needs these:

Document What to check now
NEET admit card and scorecard Correct year and your details
Class 10 certificate and marksheet Name and date of birth
Class 12 marksheet and pass certificate Marks, subjects and all pages
Identity proof An accepted ID with clear details
Allotment letter College, course, round and quota
Passport-size photos Size, number and background as stated

Read every name and date as you collect them. Make sure the marksheet has all its pages.

Keep the originals in one labelled folder. Keep photocopies in another, so they are easy to hand over. Save a private digital copy before you travel.

Folder 2: Proof for your seat claim

Some students need extra documents for a domicile, category, disability (PwBD), minority or NRI seat. Domicile means the residence status a state uses in its admission rules. NRI means Non-Resident Indian.

For each claim, check three things:

  • Who is allowed to issue the certificate.
  • Which format is required.
  • Any issue-date or validity rule.

NRI seats: start early. Proof of the sponsor’s NRI status and their relationship to you can take weeks to collect.

Disability (PwBD) seats: you usually need a certificate from a designated disability board, not a normal hospital report. See our list of NEET PwBD certificate centres.

Warning: If your proof does not meet the stated rule, your seat claim can be rejected at document verification. Only claim a seat category when your documents clearly support it.

Our MBBS counselling FAQ answers common questions about seats and quotas.

Folder 3: Your college’s own forms

Once you know your college, open its joining checklist. Look for forms such as:

  • Medical fitness certificate (often in the college’s own format)
  • Personal data form
  • Hostel admission form
  • Anti-ragging undertaking
  • Bond or undertaking, if the college or state requires one

Download the current versions before you travel. Read who must sign each form. Some need your signature, some a parent’s, and some a doctor’s. Arrange this before reporting day, not at the desk.

If the college gives its own fitness format, take that form to the doctor.

A real college checklist: Amrita Kochi

Amrita School of Medicine, Kochi, lists these for MBBS allotted candidates (general category):

  • MCC allotment letter
  • NEET UG admit card and scorecard
  • Class 12 marksheet and pass certificate
  • Class 10 certificate and marksheet (for date of birth)
  • Transfer certificate and character certificate from your last school
  • Migration certificate from your Class 12 board
  • Medical fitness certificate in the college’s format
  • Copy of Aadhaar card and copy of a parent’s PAN card
  • Caste certificate, if applicable
  • Proof of fee payment
  • 8 passport-size colour photos
  • One set of self-attested copies of all the above (self-attested means you sign each copy yourself)

What to do with this: if you are allotted Amrita Kochi, count out eight photos, make one signed copy set, and ask your school and board for the transfer, character and migration certificates now. These can take days to get. Another college may ask for a different list.

Always check the latest official notice before acting. Our seat allotted reporting checklist covers what happens at the college.

Track what is ready and what is missing

Item Needed for Who gives it Your next step
School leaving certificate Academic record School Collect the original and scan it
Migration certificate College joining Board Apply now; it can take time
Category certificate, if any Seat claim Named issuing office Check format and validity
Fitness certificate College joining Accepted doctor Book a visit with the college’s form
Bond, if required College joining College form; named signers Read the terms; arrange signatures

Add a deadline next to each missing item. Start with documents that need another office or an appointment. Copying a file takes minutes. Getting a new certificate can take weeks.

Fix name or date mismatches early

If your name or date of birth is different on two documents, write down the exact difference. Then ask the counselling authority which proof it accepts.

Be specific. For example: “My Class 10 certificate says ‘Priya Kumari’. My NEET scorecard says ‘Priya Kumari Singh’. Which document should I submit?” Use the official helpline or email.

Keep the reply with your corrected documents. Follow the official answer, not advice from a group chat.

Make clear scans

  • Scan the full page in good light. Keep seals, signatures and edges visible.
  • Open the file and read the smallest text before uploading.
  • Follow the portal’s file size and format rules. Check the quality again after compressing.
  • Use clear file names, such as “Class-12-marksheet.pdf”.
  • Preview the upload and complete the submit step.
  • Watch for any query from the verifying officer, and reply on time.

Never share your passwords or OTPs. Send personal documents only through the official portal or support channel.

Common questions

What if my original documents are with another college?

Ask that college to release them before your reporting date. Tell your new college about the situation and ask what it accepts in the meantime.

Should I carry extra copies?

Carry the number the college asks for, plus one spare set for yourself.

Can my parent report on my behalf?

Plan for the student to report in person unless the rules say otherwise. Check the reporting rules of your counselling authority and college before you book travel.
